Diabetes Nurse Educator
One (1) Full-Time Position (35hrs/wk)

West Hill Community Services (WHCS) is a diverse multi-service delivery organization providing primary health care, community/social support for individuals, families, youth, adults and seniors.  Our mandate is to promote increased access to health and wellness for the communities we serve with a focus on the broader determinants of health.

We are currently seeking a Diabetes Nurse Educator for one (1) Full-Time position for a contract from August 2010 to August 2011 to become a member of the Diabetes Education Team at West Hill. The Diabetes Nurse Educator will possess a sound knowledge base around the prevention, care and management of diabetes.  He/She will practice within an interdisciplinary Community Health Centre model of care with the ability to work independently and as a team member. Nurse Practitioner

West Hill Community Services (WHCS) is a multi-cultural service delivery agency providing primary health care, community and social support for women, families, youth, adults and seniors.  Our mandate is to promote increased access to health and wellness for the communities we serve with a focus on the broader determinants of heath.

We are currently seeking a Nurse Practitioner to provide direct clinical care and case management support for inter-agency programs and services. He/she will focus on identifying existing gaps in primary health care services and how future programming opportunities can be developed and initiated in the Scarborough area. Primary Health Care Family Physician
(Full-Time Positions)

West Hill Community Services is a diverse multi-service agency providing primary health care, community/social support for individuals, families, youth, adults and seniors. Our mandate is to promote increased awareness of health and wellness for the communities we serve with a focus on the broader determinants of health. 
We are currently seeking a dedicated Full-Time Primary Health Care Physician to assist with service delivery in Scarborough.  The PHC Physician will be an integral component of the Community Health Centre team. The Physician will have current practice experience and/or an interest in the Community Health Centre (CHC) Model of Care. He/She will deliver primary health care services within an interdisciplinary team setting working with allied and medical professionals. The Physician will be required to provide after hours on call service and outreach to clients as required. A complete job description including contact information is available.
