Community Resource Connector

West Hill Community Services (WHCS) is a diverse multi-service delivery agency providing primary health care, community/social support for individuals, families, youth, adults and seniors. We are currently seeking a Community Resource Connector for 26 hours to be a dynamic front line connector for our Hub located in the Eglinton East/Kennedy Park neighbourhood. The Community Resource Connector provides a crucial first point of contact in a busy, multi-faceted environment, as well as welcoming and orienting diverse community members, agency staff and clients, and providing other reception services. Communication, co-ordination, networking, issue identification, problem-solving and crisis management are important aspects of this job. A complete

Nurse Practitioner

We are currently seeking a Nurse Practitioner to provide direct clinical care and case management support for inter-agency programs and services. We are currently seeking a dedicated Full-Time Primary
Health Care Physician to assist with service delivery
in Scarborough. The
PHC Physician will be an integral component of the Community
Health Centre team. The Physician will have current practice experience and/or an interest in the Community Health Centre (CHC) Model of Care. He/She will deliver primary health care services within an interdisciplinary team setting working with allied and medical professionals. The Physician will be required to provide after
hours on call service and outreach to clients as required. A complete
