Community Health


The Community Health Centre provides Doctors, Nurses, Nurse Practitioners, Social Work, Dietitian Services , Diabetes Education and Foot Care services for people residing in our Catchment Area, with or without an OHIP card. Staff will maintain a comprehensive health record for each client to ensure that we are meeting the full spectrum of each client's need. All information is held in the strictest of confidence.

West Hill Community Health Centre provides on-call services to clients 24 hours a day, 7 days a week. In the event of an emergency, a physician can be contacted to provide direction and reassurance.

The Community Health Centre also provides services in partnership with other agencies and organizations, such as the Needle Exchange Program.

The Community Health Centre purpose is to provide total health care to individuals, families and community through promotion, prevention, maintenance and treatment services.

Doctors, Nurses and Nurse Practitioners provide a thorough holistic health assessment and primary health care services such as immunization, check-ups, family planning assistance and on-going medical care. Our doctors, nurses and nurse practitioners see clients who experience a particular episode of illness or injury (having the flu or spraining an ankle), and also work with clients who are dealing with on-going chronic illnesses such as diabetes. Staff endeavour to not only treat the illness or condition they encounter but to educate clients as well, to encourage an appropriate level of self-care for the given condition and to provide information to promote health and well being. Our health care staff will refer and work with specialists and in-home or hospital care providers as needed by the client.

Our Social Worker is available to provide counselling for a wide range of topics including abuse, marital discord, parenting concerns and other stressful situations. She is able to provide support for some mental health diagnosis, and also to provide advocacy for indivduals and families as needed. Our Social Worker also provides crisis intervention to those in need to stabilize and create a plan for the longer term.

Our Dietitian is available two days a week to provide counselling regarding diet for healthy living.

Our Scarborough Community-Based Diabetes Education Program provides health professional teams, experienced in diabetes care, including a Dietitian and a Nurse. A physician referral is not required. Clients may self-refer. If you would like to participate in the Scarborough Community-Based Education Program, please click here to complete and send a referral form to the Diabetes Team. Once you have completed the form, click on the "E-Mail" button on the form and it will be forwarded.

The program is for adults with Type 2 Diabetes or who are at risk of developing diabetes. Non-insured, refugees and new immigrants are welcome. No fee is required.

Counselling for indivdiuals and their families and small group sessions are available.Our staff will work in collaboration with referring physicians and primary care providers.

Group Classes are available to adults with Type 2 diabetes, Pre-Diabetes or who are Newly Diagnosed.

Education Sessions are conducted on principles of Diabetes Self-Care Management including: self blood glucose monitoring, foot care, healthy dietary principles (including meal planning, food preparation and reading food labels), fitness sessions - all delivered in an easy to understand format at convenient locations.

Individual Counselling is available for people who prefer a one-on-one discussion with the nurse and dietitian. The first visit is for 2 hours and follow-up visits are 1/2 hour long.

All programs and individual counselling sessions provide follow-up visits, before the client leaves the diabetes education program.

Our Foot Care Specialist assists with many chiropody concerns including toe nail clipping, warts and callouses and provision of special shoe orthotics.

Access Centres for Senior Citizens are offered in four area senior’s buildings (4205 Lawrence Avenue East, 4175 Lawrence Avenue East, 50 Tuxedo Court and 65 Greencrest Circuit). These Access Centres are staffed by a Nurse Practitioner who is available to provide services such as blood pressure monitoring, general health advice and instruction, and referral where needed. Needle Exchange
West Hill Community Health Centre is working in partnership with The Works (Public Health) to offer a Needle Exchange program. This service is offered Monday through Friday, from 9:00 a.m. to 5:00 p.m. at 3545 Kingston Road.
